You can schedule an appointment by calling us during normal office hours, or established patients can make an appointment using MyChart.

We offer same-day appointments for our established patients, but no walk-in services, so give us a call if you have an urgent medical need.

What to bring to your appointment

  • A valid photo identification
  • All of your medication bottles including supplements and over the counter medications
  • All of your previous records including immunizations
  • Your current address and phone number (please let us know immediately when it changes)

At your first visit you may be asked to sign a release of records form that will enable us to get your records from a previous doctor or hospital. Your previous records will help us understand some you your medical history and past treatment so we can best take care of you.

Please note if you are 15 or more minutes late for your scheduled appointment time we may have to reschedule your appointment. If you need to cancel, please call us 24 hours in advance. Multiple no-shows may result in dismissal from the office.

After visit summary

An after visit summary (AVS) is created by your provider and is given to you by their staff once the visit is complete. It usually will be given to you at check out. It is a summary of information that was discussed during your appointment. Your AVS will list the steps you have been asked to take, medication updates, and any recommendations from your provider on follow up care. Your AVS also contains patient education usually pertinent to your condition at that visit.
